CASE STUDY I - Dr. Benoit

Patient WW, 53 yo accountant. No previous CL wear. Wanted freedom from glasses for casual activities but also for daily, work activities if possible. I suggested multifocal GP CLs.
     Rx:
          OD -5.50-0.75X180
          OS -5.00-1.25X180 Add +2.00
     Ks:
          OD 43.25/44.12@ 90 dominant
          OS 43.12/44.25@ 90
     Pupils: 4mm average room light, HVID 11.5mm
     Slit Lamp: Clean lashes; lids - regular margins, no MGD; conj + pinguecula,
     nasal OU; cornea - no SPK, +arcus; iris flat, no transillumination; lens clear
     Diagnostic CLs: Essential Xtra BC 7.70 -3.00 LD 9.5 Series 2 OU
     Over-refraction:
          OD -2.50 for 20/20 Dist, +0.50 over that for 20/20 Near
          OS -2.00 for 20/20 Dist, OU
     SL: superior central position OU, good movement in all positions of gaze,
     fluorescein: slight apical clearance with good peripheral clearance
 
     Ordered CLs: Essential Xtra
          OD BC 7.70 -5.50 LD 9.5 Series 3
          OS BC 7.70 -5.00 LD 9.5 Series 3
 
At dispensing patient comfortable after 5 minutes.
     Vision: 20/20 OD,OS,OU distance and near
     SL: as above
Patient instructed on I & R and scheduled for 1 week recheck.
 
Patient has done well, wearing 12 -14 hours per day. Very happy with multifocal CLs.

 
CASE STUDY II - Dr. Benoit

Patient BC, 49 yo administrative secretary. Soft toric wearer, complaining of vision not being as sharp as with her glasses. Noting increased difficulty at near lately also. I suggested GP multifocals to correct her problems at both distances.
     Rx:
          OD -3.00-1.75X170
          OS -2.25-1.50X20 Add +1.25
     Ks:
          OD 42.87/44.62@ 85 dominant
          OS 42.75/44.25@ 100
     Pupils: 4.5 mm in average room light, HVID 12mm
     Slit Lamp: Lashes - clean; lids - mild meibomian plugs OU; conj W & Q bulbar,
     mild papillary hypertrophy tarsus; cornea - clear; iris flat; lens - clear
     Diagnostic CLs: Essential Xtra 7.70 BC, -3.00, 9.5 LD, Series 2 OU
     Over-refraction:
          OD -0.25 for 20/20 Dist, 20/20 near with same OR
          OS +0.25 for 20/20 dist, 20/20 near with same OR
     SL: OD superior central position, good movement in all POG,
     fluorescein: slight apical clearance with good peripheral clearance,
     OS slightly low position, good movement with blink but poor translation
     up on downgaze, fluorescein: apical clearance with adequate
     peripheral clearance
 
     Ordered CLs: Essential Xtra
          OD BC 7.70 -3.25 LD 9.5 Series 2
          OS BC 7.80 -2.25 LD 9.5 Series 2
 
At dispensing patient quite comfortable after 10 minutes.
     Vision: 20/20 distance OD, OS, OU; 20/25 near OD,OS and 20/20 OU
     SL: both CLs had superior central positioning, with good movement
     in all POG, fluorescein pattern slight apical clearance with good
     peripheral clearance
Patient instructed on I & R and scheduled for 1 week PE.
 
At recheck, patient very happy with vision and comfort.
