CASE REPORT
PATIENT: 14-year-old Female
HISTORY
14 year old Asian Female who recently moved into the area from another state.
Original Spectacle Rx from October 2003:
OD: -7.50 -0.25 x 040 20/20
OS: -7.25 -0.75 x 155 20/20
Goal with orthokeratology was -4.00 OU
Fitted with Fargo lenses OU for daily wear.
I first saw the patient in February 2005. Patient was wearing Fargo lenses (Daily wear of lenses):
- OD: base curve = 36.50
- OS: base curve = 37.00
Refraction:
- OD: -4.50 sph 20/30-2
- OS: -4.00 sph 20/25-1
Slit Lamp Examination:
Central corneal staining (see photo) in each eye which corresponded to the back optic zone region of the lens.
I spoke to the patient about proper care of lenses and use of gel artificial tears. I discussed with the patient and parent the need to refit the lenses to a steeper base curve to attempt to relieve some of the bearing on the cornea.
April 2005
Patient had been refit into new Fargo lenses with base curves of OD: 37.00 and OS: 37.50. There was no change in corneal staining following the change in base curves and the patient and parents were once again educated on the importance of proper care, use of AT’s and need to resolve the condition or discontinue contact lens wear.
Refraction:
- OD: -4.25 -0.50 x 150 20/30
- OS: -5.00 sph 20/30
July 2005
Upon follow-up three months later, the patient reported clearer vision with the contact lenses. Slit lamp examination revealed no central corneal staining in the right eye and significant improvement in the left eye. She reported using the gel tears four to five times daily with extra diligence in lens care. Vision was correctable to 20/20 in each eye.
